Acupuncture for Pain Management: What the Research Shows

Acupuncture has been used for thousands of years in Traditional Chinese Medicine. Western medicine has shifted from skepticism to recognition over the past two decades as clinical research caught up with clinical experience. Today, the World Health Organization recognizes acupuncture as effective for dozens of conditions — with chronic pain near the top of the list.

What the Research Says

One of the most comprehensive analyses of acupuncture for pain was published in the Archives of Internal Medicine, pooling data from nearly 18,000 patients across 29 high-quality clinical trials. The researchers concluded that acupuncture is effective for chronic back pain, neck pain, osteoarthritis, and chronic headaches — and that the benefits were not explained by placebo effects alone.

Additional studies have shown positive results for conditions including:

  • Postoperative pain: Acupuncture reduced pain scores and opioid use after surgery in multiple randomized controlled trials.
  • Knee osteoarthritis: A trial of over 500 patients found that acupuncture significantly improved pain and function compared to sham acupuncture and standard care.
  • Chronic low back pain: The American College of Physicians includes acupuncture in its clinical practice guidelines as a first-line treatment option for chronic low back pain.
  • Migraine prevention: A Cochrane review found acupuncture to be at least as effective as preventive medication for reducing migraine frequency, with fewer side effects.

How Does Acupuncture Reduce Pain?

From a biomedical perspective, acupuncture works through several known mechanisms.

Endorphin release. Needle insertion stimulates the release of endorphins and enkephalins — your body’s natural painkillers. This effect has been confirmed through imaging studies showing changes in brain activity during acupuncture treatment.

Anti-inflammatory effects. Acupuncture has been shown to reduce inflammatory markers including cytokines and C-reactive protein in both local tissues and systemically.

Nervous system modulation. Stimulating specific acupuncture points alters pain signaling pathways in the spinal cord and brain. This can turn down the volume on chronic pain signals that have become amplified over time — a process called central sensitization.

Local blood flow. Needle insertion increases microcirculation in the treated area, bringing healing factors to damaged tissue and helping clear inflammatory chemicals.

What to Expect at Your First Visit

The needles are extremely thin — much thinner than injection needles — and most patients feel only a mild sensation. Sessions last 45 to 60 minutes. Most pain conditions respond well to six to ten treatments scheduled once or twice per week.
